Imaging Case of the Week 252 Answer

The frontal shoulder x-ray shows a light bulb appearance of the humeral head indicating the possibility of a posterior shoulder dislocation.

The lateral view shows the humeral head behind the glenoid cavity thereby confirming the diagnosis.

Posterior dislocation of the shoulder is uncommon and can be missed if careful attention is not paid to the lateral shoulder view.
