A 64 year old man presents feeling unwell for 8 days. He is SOB and has a fever.

Vitals:

HR 140/min, AF (new)

BP 93/56

T 38.5

RR 46/min, His blood tests show:

pH 7.52

pCO2   25 mmHg

HCO3  20 mmol/l

O2 sats  91 RA

Na  129  mmol/l

K  3.9  mmol/l

Cl  103  mmol/l

Glucose  7.0  mmol/l

lactate  4.3  mmol/l

His CXR shows a LLL pneumonia

Describe and interpret the blood test

How would you treat him?
