The supine and rotated chest x-ray shows a large soft tissue opacity at the base of the neck with an extension into the superior mediastinum. The trachea is deviated to the left, with narrowing evident in the supraclavicular part of the trachea. This opacity is likely to be a large goitre with retrosternal extension.
