Imaging Case of the Week 480 Answer

The foot x-ray images show Lisfranc fracture.

 

There is lateral subluxation of 2 nd and 3 rd metatarsal bases. The lateral view shows a step deformity of the metatarsal bases.

Normally in the AP view of the foot, medial margin of the 2 nd metatarsal should align with the medial margin of the intermediate cuneiform.

Normally in the oblique foot view, medial margin of the 3 rd metatarsal should align with the medial margin of the lateral cuneiform bone.

Lisfranc fracture is a fracture dislocation of the tarso-metatarsals.

The patient underwent operative reduction and internal fixation.
