80 year old man presented with shortness of breath. This patient had history of CHF and COPD. His venous blood gases showed the following

  •  PH = 7.409
  • pCO2 = 54 mmHg

  • pO2 = 38 mmHg
  • HCO3 = 33.3 mmol/L
  • Hb = 86 g/L
  • Na = 135 mmol/L
  • K = 3.7 mmol/L
  • Cl = 91 mmol/L
  • Glucose = 11.5 mmol/L
  • Lactate = 1.5 mmol/L
  • Creatinine = 139 umol/L
