ECG of the Week – 24th March 2021 – Interpretation


You are reviewing a 76 year old patient who has presented after a 90 minute episode of dizzyness and shortness of breath. His GP has referred him in with an irregular pulse.

Interpretation:

  • Rate: 48
  • Rhythm: sinus rhythm
  • Axis: Normal 0-90 degrees
  • Morphology: bifid p waves
  • Intervals: increasing PR interval seen in QRS complexes 1/2 followed by dropped QRS (and seen again in QRS complexes 3/4) increasing PR interval in complexes 7/8, alternating p and QRS in complexes 5 and 6 with p with dropped QRS. QRS 100 QTc 430 (Bazzett)
  • Summary: 2nd degree AV block, alternating between Mobitz I and II
