The erect chest x-ray shows pockets of gas over the right-upper quadrant.

The patient underwent a CT scan of the abdomen, which showed a pneumoperitoneum and a right-sided sub-diaphragmatic collection. The proximal duodenum was noted as the site of perforation.

Following the diagnosis, the patient underwent a successful emergency laparotomy.
