The hand x-ray shows an intra-articular fracture of the base of the little finger. This is also known as the ‘Reverse Bennett fracture’, which is an unstable fracture and needs a referral to a hand surgeon for consideration of surgery.

Teaching point: Pay attention to the soft tissue swelling on the x-ray and this can lead eyes to the fracture site, which can be subtle as in this case.
