The neck x-ray shows

Diffuse subcutaneous emphysema in the neck.

The patient subsequently had further imaging and noted to have a pneumomediastinum with no apparent cause. Air from pneumomediastinum can travel up the neck and patient can present with sore throat, neck pain etc. Careful history & examination is important.
