Imaging Case of the Week 558 Answer

The frontal chest x-ray shows a cavity with fluid level projected over the right dome of diaphragm. There are infiltrates in the right basal region.

The lateral view shows a thick walled cavity in the posterior segment of the right lower lobe.

The patient had a fever and high inflammatory markers. The chest x-ray findings in this context are consistent with a lung abscess. The patient received appropriate treatment and a repeat x-ray after 6 weeks showed complete resolution of the findings.

The findings on this x-ray are subtle. Pay particular attention to review areas while interpreting a chest x-ray – 1. Apices 2. Hilum 3. Retrocardiac 4. Diaphragmatic.
