The foot x-ray shows

punched out lesions with overhanging edges at the head of 5th metatarsal. There is dense soft tissue swelling laterally over the 5th MTP joint. The most likely diagnosis based on the x-ray finding is gout.

The patient was treated for an acute flare up of gout with clinical improvement.
