The chest x-ray shows features highly suggestive of thoracic aortic dissection.

  • Mediastinal widening.
  • Indistinct aortic knuckle.
  • Right paratracheal density.
  • Depression of the left main bronchus.
  • Left pleural effusion.

Diagnosis was confirmed on CT aortogram and patient was referred urgently for specialist management.
