A 60 year old male presents to ED with a 1 week history nausea, decreased oral intake and occasional vomiting. He has a background of poorly controlled hypertension on  Perindopril.

pCO2 34mmHg                         Cl 107 mmol/l

HCO3 16mmol/l                        K 6.7 mmol/l

BE -8                                         Cr 607 umol/l

Lactate 2.5 mmol/l                     Glucose 5.6 mmol/l
