Lab Case 208

30 year old male presents to ED with altered level of consciousness.  According to his family he had an asthma attack in the car and then went unconscious after using his ventolin.  He has recently migrated from the Middle East

O/E Vitals within normal limits, except for RR 28 GCS E 1 V 1 M 4 – 6/15. Eyelids fluttering and carpal spasm noted. No signs of injury. Rest of exam unremarkable.

pH 7.68                               Na 142mmol/l

pCO2 14mmHg                   K 3.3 mmol/l

pO2 31 mmHg                     Cl 108mmol/l

HCO3 14 mmol/l                  Cr 94 umol/l

Glucose 6.2 mmol/l              Lactate 4.6 mmol/l
