46 year old lady, recently returned from Italy, presented with shortness of breath. Her CXR was normal. Her arterial blood gases showed the following:

PH = 7.518

PaCO2 = 28.4 mmHg

PO2 = 56.7 mmHg

HCO3 = 22.9 mmol/L

Na = 141 mmol/L

K = 5.3 mmol/L

Cl=107 mmol/L

Glucose = 6.1 mmol/L

Lactate = 2.5 mmol/L

Creatinine = 73 umol/L
