Lab case 346

72 year old man with type 2 DM, presented with diarrhoea and vomiting for 8 hours. He looked drowsy. His blood gases showed the following:

PH = 7.25

pCO2 = 50 mmHg

HCO3 = 22 mmol/L

Na = 139 mmol/L

K = 5.3 mmol/L

Cl = 109 mmol/L

Glucose = 21 mmol/L

Lactate = 2.9 mmol/L

Creatinine = 258 umol/L

Urea = 20 mmol/L
