He has nausea and poor oral intake. He presented with hypoglycemia. Sugar level was corrected on arrival, his venous blood gases showed the following:

pCO2 = 34.4 mmHg

HCO3 = 16.6 mmol/L

Na = 132 mmol/L

K = 4.4 mmol/L

Cl = 104 mmol/L

Glucose = 3.9 mmol/L

Lactate = 8.4 mmol.L

Creatinine = 340 umol/L

Ketones = 2.1 mmol/L
