A 10 year old girl presents to your ED with a 1 day history of generalised abdominal pain and nausea. On examination she looks unwell and has generalised abdominal tenderness and guarding.

1/ Raised anion gap metabolic acidosis, anion gap 17
2/ High lipase, raised GGT, elevated urea and CR
primary pancreatitis or intraabdominal sepsis (or both).
renal failure, likely pre-renal secondary to hypovolemia/shock
3/ analgesia, IV crystalloid boluses to treat hypovolemia and renal impairment, empiric abx (eg amp, gent, metronidazole).
