Imaging Case of the Week 461 Answer

The frontal wrist x-ray shows widening of the scapholunate interval. In the x-ray shown, the scapholunate interval is 5mm whereas the normal interval should be 2 mm. This abnormal scapholunate widening represents the ‘Terry Thomas Sign’. The disrupted scapholunate ligament has also caused foreshortening of the scaphoid giving rise to ‘signet ring scaphoid sign’.

 

The lateral wrist x-ray shows an abnormal dorsal tilting of the lunate, indicating the presence of DISI (dorsal intercalated segmental insufficiency).

 

The above x-ray findings were confirmed with an MRI scan and the patient underwent operative management.
