The following abdominal x-rays are from a 60 year old with abdominal pain and distension. What two radiological signs can be seen?

 

Erect AXR

Supine AXR

  1. stepladder sign (stepwise ascent of air-fluid levels LUQ) on erect axe
    string of pearls sign on erect film
    both point to SBO as the dx
    backed up by appearance of multiple dilated loops in supine film (>3cm)
