Imaging Case of the Week 242 Answer

The frontal knee x-ray shows a small elliptical bony fragment lying adjacent to the lateral tibial epiphysis; this is a Segond fracture. The importance of a Segond fracture lies in its association with severe internal knee derangement, especially ACL tears.

The HBL view shows a knee effusion, i.e. haemarthrosis in the context of trauma.

This patient had a complete tear of the ACL on the MRI and underwent ACL reconstruction.
