Imaging Case of the Week 308 Answer

The frontal elbow view shows an abnormality in the area of capitellum.

The lateral elbow x-ray shows elevated anterior and posterior fat pads. There is also a displaced capitellum fracture.

Capitellum fractures constitute about 1% of elbow fractures. Mechanism is either fall on outstretched hand or direct injury to the elbow. Undisplaced fractures are treated conservatively with immobilization.

The patient above was managed with open reduction and internal fixation.
