Imaging Case of the Week 336 Answer

The chest x-ray shows left upper lobe collapse consolidation.

There is opacity involving the left upper lobe with silhouetting of the left heart border.

There is tenting of the medial left hemi-diaphragm (i.e the juxtaphrenic peak sign) with forward displacement of the left oblique fissure on the lateral chest x-ray.

Juxtaphrenic peak sign refers to the triangular peak on the medial aspect of the hemi-diaphragm. It is caused by traction of the lower end of the major (oblique) fissure. It is secondary to upper lobe collapse.
