Imaging Case of the Week 370 Answer

The AP view of the wrist shows a widened scapholunate interval (Terry Thomas Sign) and a foreshortened scaphoid (signet ring sign).

The lateral wrist view shows DISI (dorsal intercalated segmental insufficiency) which is indicated by dorsal tilting of the lunate.

This patient was referred to hand surgeon for further management.

The frontal x-ray in this case, showed an abnormality ie scapholunate dissociation. Do not be satisfied with identifying one pathology as the lateral view indicated an additional injury (DISI) which signifies the presence of significant carpal instability.
