Imaging Case of the Week 585 Answer

The ankle x-ray shows a faint lucent line running vertically involving the distal tibial epiphysis. This is indicative of a Tillaux fracture, Salter Harris III fracture of distal tibial epiphysis.

Tillaux fracture is a fracture of the anterolateral distal tibial epiphysis due to traction from anterior tibiofibular ligament. Mechanism involved is external rotation of the ankle. It is seen in adolescent age group during closure of the growth plate which starts medially and extends laterally.
