The left thigh x-ray shows fracture of proximal femur with massive callus formation. This finding in a neonate should alert the clinician to non-accidental injury. Differential would be soft tissue/bony malignancy like sarcoma (extremely rare).

The injury pattern is an absolute indication for admission for neonate’s safety, skeletal survey and social work input.
