A 75 year old man is brought to your ED by ambulance after a fall at the nursing home he resides in. His GCs is 15 with normal vital signs.

His blood tests show:Na  178  (137-145  mmol/l)

K  4.6  (3.5-5  mmol/l)

Cl  142  (99-111  mmol/l)

HCO3  30  (22-26  mmol/l)

Urea  34.7  (2.8-7.5  mmol/l)

Creat  368  (<130  umol/l)
