82 year old lady from nursing home was brought to the emergency department with fever and shortness of breath.
Her PR = 136, RR = 30, Temperature = 38.7, Blood pressure = 91/44 and SPO2 = 90 % on 6L.
On examination she had reduced air entry right base with crepitation.
Her Blood gas shows:

K = 4.4
Urea = 12.4
Cr = 143
Albumin = 27
Lactate = 4.7
