Lab case 278

32 year old body builder presented to the emergency department complaining of muscles pain and weakness. On examination he was found to have muscles power of 3/5 in all limbs. the weakness was mainly proximal. His blood gases showed the following:

PH = 7.58

PO2 = 57 mmHg

PCO2 = 48 mmHg

HCO3 = 45.3 mmol/L

Na = 140 mmol/L

K = 1.8 mmol/L

Cl = 89 mmol/L

Glucose = 5.2 mmol/L

Cr = 103 nmol/L

Lactate = 2.4 mmol/L

CK = 17000 mmol/L
