A 21-year-old woman with history of depression. She was brought to the emergency department with recurrent vomiting and becoming drowsy.  Her VBG showed the following:

PH = 7.39

PaCO2 = 41 mmHg

PaO2 = 50 mmHg

HCO3 = 24 mmol/L

Na = 144 mmol/L

K = 3.4 mmol/L

Cl = 100 mmol/L

Lactate = 2.2 mmol/L

Cr = 53 nmol/L

BSL = 4.3 mmol/L
