78 year old diabetic man, presented to ED feeling unwell. His venous blood gases showed the following:

PH = 7.19

pCO2 = 39 mmHg

PO2 = 28 mmHg

HCO3 = 14 mmol/L

Na = 130 mmol/L

K = 4.0 mmol/L

Cl = 107 mmol/L

Glucose = 19.7 mmol/L

Lactate = 2.2 mmol/L

Ketones = 3.6 mmol/L

Creatinine = 141 umol/L

Urea = 18 mmol/L
