60 year-year-old man with history of DM and x-smoker but not known to have COPD. Presented to an emergency department with shortness of breath. His blood gases showed the following:

PH = 7.30

pCO2 = 65 mmHg

pO2 = 25 mmHg

HCO3 = 31 mmol/L

Na = 135 mmol/L

K = 4.4 mmol/L

Cl = 94 mmol/L

Hb = 134 g/L

Glucose = 13.2 mmol/L

Lactate = 1.5 mmol/L

Creatinine = 226 umol/L
