80 year old man presented with shortness of breath. This patient had history of CHF and COPD. His venous blood gases showed the following:

PH = 7.409

pCO2 = 54 mmHg

pO2 = 38 mmHg

HCO3 = 33.3 mmol/L

Hb = 86 g/L

Na = 135 mmol/L

K = 3.7 mmol/L

Cl = 91 mmol/L

Glucose = 11.5 mmol/L

Lactate = 1.5 mmol/L

Creatinine = 139 umol/L
