Imaging Case of the Week 453 Answer

Ginkgo leaf sign.

The chest x-ray demonstrates several findings –

  • Significant subcutaneous emphysema on the right with extension into the neck.
  • Mid rib fractures on the right side with pleural based opacity (likely haematoma).
  • A right sided pneumothorax.
  • Pneumomediastinum on the left.

The radiological sign seen is the Ginkgo leaf sign – air outlining the fibers of pectoralis major muscle resembling a Ginkgo leaf.
