ECG of the Week 4th December 2022 – Interpretation

There are P waves with an atrial rate of approximately 75. They are inverted in leads II, III, aVF, and V3 – V6. The P waves in aVR are upright. The origin of these P waves are atrial, but not from the sinoatrial node. The ectopic atrial rhythm was likely conducting through the AV node at a rapid rate, resulting in tachyarrhythmia. With adequate AV nodal blockade, the ventricular rate became more controlled. The QRS complexes are narrow and there is ST segment depression inferolaterally. The patients troponin was not elevated and the presentation did not resemble ACS.
